Where does serana go when lost?

If serana isn’t at any of the two castles while your doing the unseen visions quest, then she will be at Ancestor Glade entrance. That’s where I found her when I lost her.

What happens to serana after Dawnguard?

If you complete the full dawngaurd quest and defeat Harkon, she says she’s going to become human and she goes to Falions house in mortal. She’ll tell you she’ll come see you when she’s done and will return to the dawnguard later.
